HIPAA and Privacy Policy

All patients will be required to review and sign a form regarding the protection of their HIPAA and private information before receiving treatment.

Communication: Healthy Mind Clinic may need to contact you by phone or email about appointment reminders or request pertinent information regarding services provided. We also will request the ability to communicate through text messaging/SMS. Text messaging/SMS consent will be requested upon the intake process and remain private from the general public.
